MCAS Academic Forms
Below is a list of forms most frequently used by undergraduate students in the Morrissey College of Arts and Sciences.

MCAS Undergraduate Academic Forms
This form should be used if you are taking a course outside of Boston College (example: taking summer courses at another institution). |
This form should be used to withdraw from a course by the published course withdrawal deadline each semester. |
This form should be used if you require an excused absence for a class due to extenuating circumstances. Your request will be reviewed by your academic dean and you will be notified via email if your request has been approved or denied. |
| Internal Transfer Form - Morrissey College of Arts and Sciences This form should be used if you are currently enrolled in another school (CSOM, CSON, LSEHD, etc) and you wish to internally transfer into MCAS. |
This form should be used if you would like your internship to count for 1 credit toward your Boston College degree. |
This form should be used to drop a course (not withdraw or add) after the add/drop period. This form must be submitted by the published late drop deadline each semester. |
|  —First-Years Only First-year students can use this form to declare their major as long as the major does not require an application. Sophomores, juniors, and seniors must visit individual academic departments to declare or change majors. |
This form should be used if you are currently on a Leave of Absence and you would like to return to Boston College. |
| Seal of Biliteracy Form (COMING SOON) This form should be used if you took a proctored exam and earned the Seal of Biliteracy for a language. You must submit this form by August 1st before you begin your first semester at Ïã½¶Ðã if you would like the Seal of Biliteracy to fulfill your MCAS language requirement. |
This form should be used if you wish to withdraw from the University or take a Leave of Absence (personal or medical). |
